I, hereby declare that I am voluntarily sharing my Aadhaar number and demographic information issued by UIDAI with Triaina Health for the exclusive purpose of creating an ABHA number. I understand that my ABHA number may be utilized and disclosed for objectives that could be notified by the ABDM, including but not limited to, the delivery of healthcare services. I am aware that my identifiable personal information, such as Name, Address, Age, Date of Birth, Gender, and Photograph, may be made accessible to entities within the National Digital Health Ecosystem (NDHE). This includes stakeholders like healthcare professionals (e.g., doctors), facilities (e.g., hospitals, laboratories), and data fiduciaries (e.g., health programs) that are registered with or connected to the Ayushman Bharat Digital Mission (ABDM) and its related processes. I authorize Triaina Health to use my Aadhaar number for Aadhaar-based authentication with UIDAI under the Aadhaar (Targeted Delivery of Financial and Other Subsidies, Benefits, and Services) Act, 2016 for the stated purpose. I understand that upon successful authentication, UIDAI will convey my e-KYC details, or a response of “Yes,” to Triaina Health. Despite being informed of the alternative ID options available, I choose to use my Aadhaar for availing benefits within the NDHE. I recognize that my personal identifiable information, excluding the Aadhaar or VID number, may be utilized and shared for the aforementioned objectives. I retain the authority to withdraw this consent at any moment, in accordance with the provisions of the Aadhaar Act and its regulations.
